Review the Advertising

Submit suggested changes to your Manager along with a completed Advertising Submission Form (F5985). Your manager will forward to UA’s Home Office
for approval. |

Ad cannot be used until you
have received final approval from the Compliance Department
at
UA’s Home Office. Allow 3 weeks (or more) to process
depending on type of ad and state regulations. |

If Compliance requires any
changes to the ad, it is your responsibility to make the
changes and resubmit to UA’s Home Office. |
| 5. |
Once you receive final
approval from Compliance, you are ready to use the ad. |

A tear sheet of the ad,
showing name and date of publication, must be provided to
UA’s Home Office for company records. |
| 7. |
Due to changing ad guidelines,
resubmit the revised approved ad to UA’s Home Office for
reapproval every 60 days, if you continue to run
the ad. |
| 8. |
Advertising meetings are held
every Wednesday at UA’s Home Office. To expedite the
approval process, submit your ad to Compliance by Monday
morning in order to be reviewed that week. Submissions
received after noon on Monday will be reviewed the
following week. |
