Plan codes can be found on the Texas ProCare rate card. Please take care to legibly write in the plan codes. Upon reprint we will include preprinted plan codes on the apps.

Kansas has approved the Good Sense Plan
(GSP). Order your supplies today and they will be shipped upon completion of printing.
Delaware now requires mental illness coverage for the
GSP. Revised rate cards are being printed and will be available to order upon completion.
